Hospice Care of Southwest Michigan

Who We Are

For 35 years we have provided compassionate, professional end of life care in southwest Michigan.  In that time, we have learned that it is important to support people wherever they are in the journey.  Attending to the end of life is just as important as nurturing the beginning.  Hospice Care of Southwest Michigan is a community based, non profit provider of hospice services, serving Kalamazoo, Calhoun, Van Buren, Barry, Branch, St. Joseph, Cass and Allegan counties.

What We Do

Our Services
Since 1981, Hospice Care of Southwest Michigan has been providing hospice services to patients and their families in our community.

·  Registered nurse on-call availability 24 hours a day, 7 days a week
·  Physician services

·  Spiritual care services
·  Grief support services
·  Home health aide/homemaker services
·  Volunteer services
·  Prescription medications related to the terminal  illness
·  Medical equipment and supplies related to the terminal illness
·  Respite care
·  Continuous crisis care
·  Physical, speech and occupational therapy
·  Payment accepted from all insurance plans, including Medicare and Medicaid

The special care we offer.

Admissions
We respond to your requests within hours so you have confidence that we will follow through.  We admit the same day, evenings and on weekends.
Hospice physicians 
Our professional team includes hospice physicians certified in Hospice and Palliative Medicine who are available to make home visits.
Medical treatments 
We offer individualized medical treatments to ensure your patients’ comfort and quality of life (blood transfusions, intravenous hydration, paracentesis, wound consultation, palliative radiation, lab, etc.)
Financial assistance 
We serve everyone, regardless of the ability to pay, and offer help with financial issues that often arise during a serious illness.
Life story recording 
We offer your patients the opportunity to record their life stories– a valuable gift of memories for their loved ones.
Massage therapy 
We offer massage therapy to provide relaxation and relief to sore and stiff muscles.
Music therapy 
We offer music therapy to help reduce pain and anxiety, decrease isolation and strengthen family bonds.
Pet therapy 
We offer pet visitation with certified therapy dogs to ease anxiety, reduce stress and increase socialization.
Grief and Loss Services 
We offer counseling and support groups for families and friends during and at Oakland Centre after illness.  Special grief support services are available for children and grandchildren.
Rose Arbor and Glenn Arbor
We set the standard of care for residential and inpatient hospice services with private rooms to provide clients and their families with the specialized care they need.
Adult Day Services 
Our professional staff offers comprehensive services that allow caregivers at Oakland Centre a much-needed break.  They can handle personal business or just relax, knowing their loved one is well cared for and safe.
